Skilled Nursing

Nursing that never clocks out

Nurses are on duty 24 hours a day, every day, managing medications, watching chronic conditions and catching small changes before they become big problems.

  • Medication management in coordination with physicians and pharmacy
  • Diabetic care, including monitoring and dietary support
  • Wound care and post-surgical monitoring
  • Long-term care for residents who need ongoing skilled support

Respiratory Care

Ventilator & trach care, minutes from home

Not every skilled nursing facility can care for a resident who depends on a ventilator or tracheostomy, which can mean long drives for families. Riverside provides that respiratory care right here in Arkoma.

  • Ventilator management by a team trained in respiratory care
  • Tracheostomy care, including routine trach care and suctioning
  • Coordination with physicians and the Fort Smith medical community
  • Visits in minutes, not road trips, which matters for recovery too

Rehabilitation

Therapy with one destination: home

After surgery, a fall or a hospital stay, our physical and occupational therapists build a plan around your goals, then work it with you day by day.

  • Physical therapy to rebuild strength, balance and mobility
  • Occupational therapy for the everyday skills that make independence possible
  • Goals your family can follow and ask about at any time
  • Medicare coverage for qualifying rehabilitation stays

Skilled nursing rehab is different from an inpatient rehabilitation hospital, which is built for people who can take part in intensive therapy, generally about three hours a day. If that would fit your loved one better, we’ll tell you honestly.

Questions Families Ask

Straight answers, plainly given

Does Riverside accept Medicare and Medicaid?

Yes. Riverside Health Services is certified for Medicare and for Oklahoma Medicaid, which is called SoonerCare. The nursing home does not decide Medicaid eligibility: in Oklahoma you apply through your county Oklahoma Human Services (OKDHS) office, as explained by the Oklahoma Health Care Authority. Our team will help your family understand the steps before admission. Medicare Advantage plans set their own rules for skilled nursing stays, and many require approval before admission (prior authorization). For Medicare Advantage or other insurance, call us with the plan name and we’ll check for you.

My parent lives in Arkansas. Can they use Medicaid at Riverside?

Medicaid is run by each state, and in most cases Arkansas Medicaid does not pay for long-term care in an Oklahoma nursing home. A person who moves to Riverside would apply for Oklahoma Medicaid (SoonerCare) through Oklahoma Human Services, which decides eligibility. Call our Business Office Manager at (918) 875-3107 before the move so we can walk your family through the timing. See Admissions & Costs for more.

Do you provide ventilator and tracheostomy care?

Yes. Specialized ventilator management and tracheostomy care are core services at Riverside, which means families in the Fort Smith area can keep loved ones with complex respiratory needs close to home.

How long is a typical rehab stay?

It depends on the person and their goals. Short-term rehabilitation after surgery, injury or a hospital stay often runs a few weeks. Medicare can cover up to 100 days of skilled nursing care for qualifying stays; we’ll help you understand what applies to you.

What is the difference between short-term rehab and long-term care?

Short-term rehab focuses on therapy and recovery with the goal of returning home. Long-term care is for people who need skilled nursing and daily support on an ongoing basis. Riverside provides both.
